Questions About Insurance 

It is the patient's responsibility to provide our Patient Accounts Department with required insurance information: 

We need the address of the insurance company, your policy number, the type of policy (family or single) and the effective date for the insurance coverage.  Your Social Security number is also required.

 

Medicare has two billing parts. Medicare Part A covers inpatient hospital services.  Medicare Part B covers clinic services, physician fees, and outpatient hospital services.

 

Your employer or insurance agent can provide you with coverage information.  Details on Medicare benefits are available at your local Social Security office.
