What Is Advance Care Planning?

If you suddenly became unable to communicate, your family members would be asked to make numerous medical care decisions for you.

We have Living and Durable Power of Attorney for Health care and we have a Notary Republic in the bulding Monday - Friday. 

 

Advance Care Planning is a process that establishes your desires about future care in the event that you are unable to participate in decisions.  The Advance Care Planning department at Jacobson Memorial Hospital can assist you and your family with your planning needs.
